In 2018-2019, at the Keck School of Medicine of the University of Southern California, we developed and piloted a narrative-based health systems science intervention for patients living with HIV and medical students in which medical students co-wrote patients’ life narratives for inclusion in the electronic health record. The pilot study aimed to assess the acceptability of the “life narrative protocol” from multiple stakeholder positions and characterize participants’ experiences of the clinical and pedagogical implications of the LNP. Students were recruited from (...) KSOM. Patients and staff were recruited from the Maternal, Child, and Adolescent/Adult Center for Infectious Disease and Virology at Los Angeles County+USC Medical Center. Ten patients, seventeen students, and ten MCA staff participated in the pilot study. Qualitative methods were used to gather data from students’, patients’, and staff’s perspectives. Three themes emerged from the thematic analysis: patients’ life narratives conveyed their unique life experiences and voices; the protocol could result in wide-ranging effects on HIV care; the LNP enabled students to contribute value to patients’ healthcare. Across groups, participants considered the LNP an acceptable intervention. The LNP, its limitations, and implications for HIV care, narrative medicine, and health information technology are presented. (shrink)

In moral case deliberation (MCD), healthcare professionals meet to reflect upon their moral questions supported by a structured conversation method and non-directive conversation facilitator. An increasing number of Dutch healthcare institutions work with MCD to (1) deal with moral questions, (2) improve reflection skills, interdisciplinary cooperation and decision-making, and (3) develop policy. Despite positive evaluations of MCD, organization and implementation of MCD appears difficult, depending on individuals or external experts. Studies on MCD implementation processes have not yet been published. The (...) aim of this study is to describe MCD implementation processes from the perspective of nurses who co-organize MCD meetings, so called ‘ local coordinators ’. Various qualitative methods were used within the framework of a responsive evaluation research design. The results demonstrate that local coordinators work hard on the pragmatic implementation of MCD. They do not emphasize the ethical and normative underpinnings of MCD, but create organizational conditions to foster a learning process, engagement and continuity. Local coordinators indicate MCD needs firm back-up from management regulations. These pragmatic action-oriented implementation strategies are as important as ideological reasons for MCD implementation. Advocates of clinical ethics support should pro-actively facilitate these strategies for both practical and ethical reasons. (shrink)

This paper reviews a number of huge challenges to ethical leadership in the twenty-first century and concludes that the need for global ethical leadership is not merely a desirable option, but rather – and quite literally – a matter of survival. The crises of the recent past reveal huge, and in some cases criminal, failures of both ethics and leadership in finance, business and government. We posit that mainstream economic theory’s construct of ‘homo economicus’ and its faith in the ‘invisible (...) hand’ of the market constitute deeply flawed foundations upon which alone policy may be built and, farthermore, that these problematic foundations exert substantial shaping power over the institutional and discursive landscapes in which international business is transacted. Analogously, we argue that dominant approaches to business ethics and corporate social responsibility are, if not incorrect, at least in need of revisiting in terms of questioning their basic assumptions. Instead of the smugness of Western (especially Anglo-American) attitudes towards other ways of thinking, valuing and organising, it appears clear that openness, cooperation and co-creation between the developed and developing worlds is a basic prerequisite for dealing with the global challenges facing not just leaders, but humanity as a whole. This objective of stimulating discussion between dominant and marginal voices has guided our selection of papers for this Special Issue. We have thus included not only representatives of research from within the parameters of mainstream business ethics, IB or leadership scholarship, but also innovative contributions from fields such as military history, information technology, regulation, spirituality and sociology. "A distinctive voice somewhere between Mark Twain and Michel Montaigne" is how Psychology Today described A.C. Grayling. In Life, Sex, and Ideas: The Good Life Without God, readers have the pleasure of hearing this distinctive voice address some of the most serious topics in philosophy--and in our daily lives--including reflections on guns, anger, conflict, war; monsters, madness, decay; liberty, justice, utopia; suicide, loss, and remembrance. A civilized society, says Grayling, is one which never ceases having a discussion with (...) itself about what human life should best be. In this book, Grayling adds to this discussion a series of short informal essays about ethics, ideas, and culture. A recurring theme is religion, of which he writes "there is no greater social evil." He argues, for instance, that liberal education is better than religion for inculcating moral values. "Education in literature, history, and appreciation of the arts," he says, "opens the possibility for us to live more reflectively and knowledgeably, especially about the nature and variety of human experience. That in turn increases our capacity for understanding others better, so that we can treat them with respect and sympathy, however different their outlook on life." Thought provoking rather than definitive, these essays don't tell readers what to think, but only note what has been thought about how it is best to live. A person who does not think about life, the author reminds us, is like a stranger mapless in a foreign land. These brief and suggestive essays offer us the outlines of a map, with avenues of thought that are a pleasure to wander down. (shrink)

Nash Teaching about Ferguson: An Introduction This forum was organized around the idea of asking feminist scholars to reflect on the practice of teaching about racial violence as well as on the experiences of teaching in the midst of racial violence. What do feminist pedagogies centered on Ferguson and (...) its aftermath look like? How do we present the various forms of violence—including state action (in the case of murder) and state inaction (in the case of nonindictments)— that produce and uphold the conditions that mark the current situation? How do we bring our feelings about this moment into our classrooms, and how do we do this feeling-teaching in ways that attend to the fact that feminist scholars are endlessly called on to perform affective labor and also that racialized and gendered bodies’ affects are policed inside and outside of the academy? What happens if we refuse the composure that faculty bodies are supposed to perform and enact grief, rage, or sadness? What happens if we refuse performing anything but exhaustion, numbness, or a protective desire to shield our bodies from our students’ scrutiny or curiosity? It quickly became apparent that a forum focused on “feminist pedagogies of Ferguson” would be far more expansive than a conversation focused on how to teach about Ferguson; put simply, to speak about Ferguson is always to speak about more than Ferguson. “Ferguson” has become shorthand for a murdered young man, Michael Brown, for a grand jury’s nonindictment of the police officer who shot him, and for 212 Jennifer C. Nash the Department of Justice’s recommendation not to bring civil rights charges against the officer, Darren Wilson. “Ferguson” has also become shorthand for a number of lives that ended violently, often at the hands of the state: Eric Garner, Oscar Grant, Trayvon Martin, Tarika Wilson, Tanisha Anderson, Renisha McBride, and Tamir Rice, to name a few. “Ferguson,” then, has become a keyword around which scholars organize conversations about legal murders, the variety of laws that regard property more than black life (so-called stand-your-ground laws, for example), racial profiling, stop and frisk, mass imprisonment, the death penalty, and the racial logics of systems of value that ensure that racially marked bodies are, in Lisa Marie Cacho’s words, “ineligible for personhood.”1 The scholars included in this forum capture the expansive ways that we, as scholars and educators, articulate the meanings of this moment and the ways that we situate Brown’s death in the conditions of the unfolding present. The scholars included in this forum also powerfully ask about the possibilities for activism in the wake of Ferguson(s) and the powerful coalitions that have formed in the wake of Ferguson(s). It is provocative in that [End Page 168] she appears to be doing buddhology even though she resists calling it that. She says she doesn't want to (...) take the voice of a buddhologian. She does not describe what is as a phenomenologist might or what was as an historian of religion might, or what should be as a buddhologian might. She describes what might be, as a—well, as a participant-philosopher might. She writes as an outsider taking the voice of an insider.Kiblinger chooses as her topic the relationship of Buddhists to people of other religions. She begins by noting that Buddhists themselves have not explicitly addressed this topic in their religious philosophizing, at least not to the extent Christians have. But, she suggests, implicitly they behave as if they do have a favored response to this issue. Citing a wide variety of Buddhist sources across the Buddhist spectrum, she concludes that inclusivism is the most natural position for Buddhists to take vis-à-vis the other religions. She describes that position in some detail, all the while persisting in her refusal to be considered a spokesperson in the insider sense.Kiblinger draws her examples from the way two historical figures related to people of non-Buddhist ideologies and from historical contacts Buddhism as a movement had with other religions or sects. The first historical figure is Siddhartha Gautama himself. The Buddha, Kiblinger notes, was not shy about identifying what made any religion (not just his dhamma) useful—the noble eightfold path. This path is the core of Gautama's dhamma, but it also appears in other religions in other forms. The Buddha put forth the presence of this content as the measure of a religion's usefulness (see the Mahaparinibbana Sutta). And of course the parable of the raft relativizes all religious teachings, making them true only to the extent that they enable humans to "cross the stream" of temporal existence, a common tenet of the inclusivist position in all its forms.Asoka represents a second example of Buddhist inclusivism for Kiblinger. In the seventh and twelfth of his Rock Edicts, Asoka makes clear that all members of all religious traditions are welcome to practice in his kingdom, even though he thinks Buddhism is the best position. He makes clear that both exclusivism and intolerance of other religious ideas are not acceptable positions in his way of looking at the spiritual dimension of the world. Existentially, Asoka's life story is a model of a reformed "exclusivist" finally embracing the all-truth-no-matter-where-you-find-it-is-still-truth position.Not just these giant figures model inclusivism. Buddhism as a movement has consistently, Kiblinger argues, behaved toward other movements as inclusivists would. She uses as a first example the Buddhist attitude toward Vedic sacrifice, the religious practice most liable to be cast aside by the new teaching of Gautama. We see in this example, Kiblinger says, "the technique of significant reinterpretation and new application rather than subordination" (p. 41). She also notes that a technique she calls "reinterpretation by ethicization" that does not throw out Vedic teaching altogether but claims at its root a teaching that was originally Buddhist.Evidence for the inclusivist position can also be found in the way Mahayanists treated Theravadins, or Hinayanists as Kiblinger refers to them. Besides the obvious intellectual satisfaction of solving a difficult diagnostic problem or the ability to make a comfortable living, I have found the greatest personal sense of moral gratification when helping [End Page 196] families negotiate the most challenging event in life: making decisions at end of life. Whether the (...) condition was a ruptured intracranial aneurysm with brain death or a patient with far advanced cancer struggling with the choice between experimental chemotherapy or choosing comfort therapy at home surrounded by family for the remaining days of life, the moral courage required to provide the best guidance was considerable. In my early years of practice I didn’t call my actions in these situations “courageous” nor were there moral “guidelines” providing a blueprint. I knew I responded with empathy and compassion and that this ability had a lot to do with being raised in a religious or spiritual home. At the same time, I suspected that some of the lack of empathy and compassion I witnessed in some of my colleagues was related to the very training programs of physicians. This essay traces the origin of my moral and ethical concerns about medical education and my intention to reinvigorate the spiritual or existential voice in medicine.An early life experience had a great deal to do with shaping my moral development. At age seven my family traveled through the West Indies. My father’s amateur radio hobby meant that at every stop we would be greeted by one of his radio contacts. One stop in particular was life changing.Port au Prince, Haiti was an unknown place to this seven year–old, but to say it was a culture shock to a boy from Kansas would be an understatement of epic proportions. It was during this visit that I experienced true poverty in its most blatant form. In downtown Port au Prince I saw limbless beggars and the deformed from polio and malnutrition. But it was the sight of the garbage dump near the center of town that changed my world forever. There, among piles of detritus, I saw children and families making their home in the dump. They had hollowed out a sort of cavern in the piles of trash that served as a place of refuge from both the rain and the sun.This experience in many ways has served as a framework and background for my life. Although seen through the eyes of a seven year–old, the picture can only be adequately understood within the context of being raised by religiously faithful parents, parents free of hypocrisy whose lives personified the best in Christian virtues and practices. These early experiences planted the seeds that would only fully blossom some 60 years later.Medical school was a demanding but exhilarating experience. Each day was a day of discovery … anatomy, physiology, pathology … all subjects opening up a whole new world to explore. Internal medicine suited me best since it was the specialty that most closely integrated all the basic sciences necessary to arrive at a diagnosis. I loved the elegance of endocrinology in particular where a given hormone mediator could cause widely different effects if present in excess or completely absent. But I was in the end drawn to cardiology since it combined complex physiology with my avocation, the love of running.There was, however, a disturbing aspect of medicine education that I observed affecting many of my classmates. Although I can vividly recall the enthusiasm that all of us shared as we donned our white coats for the first time and envisioned a future of caring for our patients, things gradually and insidiously changed over the next four years. The altruism and empathy that was so evident in the early years gradually lessened as graduation approached. I could not help being struck by this change and wondered at the time why that should happen. Little, if anything, in Justin scholarship has been as controversial as the dating of the so-called Epitome of the Philippic History of Pompeius Trogus. Suggested dates have varied from the time of Antoninus Pius through the third century to the end of the fourth. The latter was proposed in 1988 by Sir Ronald Syme, but has in fact received little support in subsequent literature on Justin, which has tended to accept the earlier dating . An exception is T. D. Barnes, (...) who has voiced support for a later dating based on a linguistic parallel to Justin in the Historia Augusta. Barnes observes that Oscar Hey, author of the article ducatus in TLL , drew attention to the similarity between Justin 30.2.5 Agathocles regis lateri iunctus civitatem regebat, tribunatus et praefecturas et ducatus mulieres ordinabant and HA Heliog. 6.2 militaribus… praeposituris et legationibus et ducatibus venditis. Hey, he notes, refers at the head of that particular section of the article to Seeck's article on dux. Seeck had demonstrated—and this is now taken for granted by scholars—that from the time of Diocletian dux is used technically as a formal title, and, in drawing attention to the parallel between HA Heliog. 6.2 and Justin, Hey must have been intimating that Justin is, like the author of Heliog. 6.2, using ducatus as a formal title. Philosophy has had its share: one thinks of Wittgenstein and C. S. Peirce, but many lesser Wittgensteins and Peirces somehow (...) adapted and became tenured. In regard to such figures “eccentric” meant outside the circle or off center; “radical” meant surprising, original, sincere. Today radical has a murky political sense. Most radicality takes place in a discourse, or as a discourse—in the title of a university press book—which when invoked aligns the writer with the orthodoxy of a speech community. No surprise; only a dull echo as membership is [End Page 516] confirmed and the circle remains unbroken. Contrary to almost everyone’s intentions, never has so much “identity politics” been melded with the writing act in a metaphysical proof. We write about pragmatism, therefore we are new pragmatists. We write eco-feminism, therefore we are radical eco-feminists.Yet the nonconformist self-image of many philosophers is belied less by the uniformity of their discourse than of their lives, constrained as they are by professional protocol. This situation has become especially painful for phenomenologists whose credos “to the things themselves” and “towards the concrete” mandate a method of worldbound description confirmed by first-person evidence. It is a longstanding embarrassment that most phenomenological studies are either studies about phenomenology, or phenomenologies of books. In this regard even before Of Grammatology it was obvious that there was nothing outside the text.The rare exception is Alphonso Lingis, author of Excesses (1983), Libido: The French Existential Theories (1985), and The Community of Those Who Have Nothing in Common (1994). So much is obvious to anyone who leafs through his Abuses. The vistas and catastrophes we access through this book cannot be easily disengaged from the personality that guides us to Tenochtitlan, Matagalpa, Kuta, Bali, the Sambodromo in Rio de Janeiro, Khlong Toei, and Chichicastenango. From these places and others Lingis reports the sometimes macabre longings and vulnerabilities of those who have nothing in common except violent changes in heritage and landscape. He speaks to their wounds and of his own ailments he acquires by exposing himself to forsaken circumstances: it is all material for his philosophy. In this work speech is primary, as it was for phenomenologists before Derrida.Merleau-Ponty’s writing remains unsurpassed as an eloquent transcript of perceptual life as expression and bodily engagement. Lingis, who translated The Visible and the Invisible, has from the start of his career been the most interesting and outlandish promulgator of this style of phenomenology. But his focus was decisively turned by Emmanuel Lévinas (whom Lingis has also translated) towards alterity relations, intimate events in which persons defer to and ennoble strangers by taking responsibility for them. Speech and physical contact are precious components of these relations, allowing persons to disburden themselves, to exchange pain and betrayal for tenderness and trust.Lingis shows how alterity relations suffer abuses at the face-to-face level and in the global network of electronic media. We enter his work breathlessly, as into a deep valley where there is gunfire and tropical birds. Spy satellites pass overhead. The human beings are strangely quiet. Women and children are being herded into canvas-covered trucks by youthful militia with oversized weapons. The volatile prose lifts up the voices of the damned just before they vanish around the bend in the jungle. Lingis speaks for them. This is the mandate of his research: “One has to speak for the silenced” (p. 36).How does highly educated thought speak for the tortured and dispossessed [End Page 517] without looking absurd or merely pretentious? Professor Cameron's keenly anticipated argument (outlined in TAPA 122 (...) [1992] 305-12) does not disappoint, especially now that he includes among the "critics" not only such ancient "Telchines" as Asclepiades and Posidippus but also the Roman Callimacheans and "moderns with Hesiodic initiations on the brain" (415). To counter "the curious modern preoccupation with Hellenistic epic and epyllion" (ix), seen most starkly in interpretation of Catullus and the Augustans, Cameron interweaves two ambitious projects.First, the organizing argument is a reconstruction and dating of the Aetia so as to establish that the prologue (frag. 1 Pf.) introduced books 1 and 2 ca. 270 [End Page 339] B.C. amid debates about contemporary elegy. In place of the now generally discredited "feud" with Apollonius over monumental epic, Cameron posits a quarrel with Asclepiades and Posidippus over the epicizing Antimachean mode of elegy. The prologue is not a recusatio of grand epic (a Roman preoccupation); indeed, "... there is no mention of epic in the Aetia prologue" (457), and elegy would be a curious medium for dictating taste in hexameter. Nor does the prologue claim novelty for the Aetia as catalogue-style "Hesiodic elegy," which had been in fashion since the Lyde, and indeed so much so that "it was pseudo-Hesiodic rather than pseudo-Homeric writing that aroused Callimachus's ire" (363). The "Telchines" are the admirers of the Lyde, a poem that Callimachus assails as an "epic pastiche" (483), a monotonous, and the stylistic antithesis of the interesting subjectivity and personal voice demonstrated in his own Cydippe (ca. 279-274). For having compromised the distance of elegy from epic, Antimachus is left pointedly unnamed next to Mimnermus and Philitas (316) and further overshadowed by allusion to Aratus (in, frag. 1.11) as the proponent and paragon of. In sum, the prologue's polemic lies more completely within the moment and between the lines than has been suspected, and entirely within elegy but outside of theory: "So Callimachus's famous literary theories do not really amount to much-as theories" (452).Second, to understand how the Aetia speaks for its age we must understand the age rightly. To provide "a prolegomena to the study of Hellenistic (and so also Roman) poetry" (ix) Cameron pauses frequently in his reconstruction of the Aetia to launch diatribes against the folly of reducing Hellenistic poetry to the aestheticism and obscurantism of the "ivory tower," or to genre inversion (contra Rossi), Buchpoesie (contra Bing), or, worst of all, some "contest of Homer and Hesiod" about big and small epics. This rarefied "Alexandria" existed first in Rome and at a great remove from the continuities of Hellenic culture that shaped Callimachus' "real literary context, a world of cities as well as courts, a world of private symposia and public festivals" (ix). Multibook epics were too rare (contra Ziegler) to require recusatio, while the popularity of short hexameter poems precluded controversies such as are implied by the term of "epyllion" (contra Gutzwiller) or imputed to Theocritus (Id. 7, 13, and 22) and Callimachus (H. Apollo and Hecale). "To judge from the debate about the Lyde, it was elegy that preoccupied the more reflective poets and critics of the age" (451).Only a scholar with Cameron's range, good ear, and courage could undertake a two-pronged offensive of this sort, which is not less "original and polemical" than the elegy it celebrates. Recently published papyri, Byzantine misreaders, and the shifty tribe of epigrammatists have all been marshaled with equal authority for what is in part the elegiac pre-history to his distinguished The Greek Anthology from Meleager to Planudes (Oxford 1993). By the nature of the fragmentary evidence, both the elegy-centered age and its important (if not definitive) Aetia can be reconstructed only at the level of hypothesis, and readers will differ as... It is now generally agreed that in Aristotle's Poetics, ch. 13 means ‘mistake of fact’. The moralizing interpretation favoured by our Victorian forebears and their continental counterparts was one of the many misunderstandings fostered by their moralistic society, and in our own enlightened erais revealed as an aberration. In challenging this orthodoxy I am not moved by any particular enthusiasm for Victoriana, nor do I want to revive the view that means simply ‘moral flaw’ or ‘morally wrong action’. I shall (...) try to show that the word has a range of applications, from ‘ignorance of fact’ at one end to ‘moral defect’, ‘moral error’, at the other, and that the modern orthodoxy, though not as clearly wrong as the moralizing interpretation it displaced, restricts Aristotle's meaning in a way he did not intend, and does lessthan justice to his analysis of classical drama. The strange series of hypodochmiacs here and atO.T.1207 ff., with brevis in longo without pause atAj.421 andO.T.1208, seems metrically self-contained, despite their syntactical interdependence (esp.Aj.421–2οὐκέτ' ἄνδρα μὴ | τόνδ' ἴδητ', so that the word-overlap ofοἷονinto iambics in Pearson's text is unlikely.ἑξερῶ μέγαshould therefore be writtenplena scriptura. Thenοἷον οὔτιν' ἁ Τροί|α στρατοῦ…is possible, but the ithyphallic with word-overlap, sometimes found in the syncopated iambics of Aeschylus, is foreign to Sophocles. If our theology about the human condition correctly underscores our brokenness, and our understanding of the world as being fallen indeed reflects reality, then our poetry of worship should express that brokenness as well as our longing for healing. However, the Church in North America neglects laments because we have essentially lost the art and practice of grieving in the West, the Church silently condones some bad theology about Christian living, and we lack a robust philosophy of language in Christian (...) practices and disciplines. It has long been accepted as a principle by editors and writers on Greek metre that brevis in longo and hiatus in tragic lyrics often coincide with some kind of sense-pause. The object of this inquiry is to determine the incidence of pause in such places, and show that it is significantly high; to show that there is a comparable incidence in the corresponding places in strophic systems; to show that period-ends determined by criteria other than brevis and hiatus are (...) attended by similar conditions. It might seem that if all this were true it would have been recognized long ago, particularly as the connection between sense and metrical structure, and symmetry of sense in strophe and antistrophe, has often been pointed out.
